Our RN Case Managers play a critical role in ensuring patients' final days, weeks, and months are as comfortable as possible.

• Conduct initial client assessments and continuously refine care plans.
• Provide hands‑on nursing care and patient education.
• Maintain open lines of communication with patients, families, and the healthcare team.
• Manage patient care coordination for your caseload.
• Participate in interdisciplinary team meetings to ensure comprehensive care planning.

• Licensed as a registered nurse (RN) in the state you work in.
• Proficiency with electronic medical records (EMR) software.
• Excellent planning, organizing, and communication skills.
• Ability to solve problems autonomously, ensuring responsibilities are met while maintaining high standards.
• Valid driver’s license and reliable vehicle with current insurance.
