. You’ll work closely with patients and families, forming deeper relationships and delivering individualized support that goes beyond traditional clinic-based roles.

• Provide direct nursing care in accordance with the patient’s care plan and under the supervision of an RN Case Manager.
• Deliver skilled services such as medication administration, wound care, and monitoring of vital signs.
• Collaborate with physicians, RNs, and other members of the interdisciplinary team.
• Document patient care thoroughly and promptly using provided technology.
• Educate patients and caregivers on health conditions and home care routines.

• Current LPN license in good standing in North Dakota.
• CPR certification (we’ll provide if needed).
• Valid Driver’s License and reliable transportation.
