• Evaluate patients' physical function and develop individualized treatment plans to achieve optimal outcomes
• Provide physical therapy interventions, including exercises, stretches, and modalities, to promote patient independence and comfort
• Collaborate with interdisciplinary teams to develop and implement comprehensive treatment plans
• Communicate effectively with patients, families, and healthcare providers to ensure seamless care transitions
• Maintain accurate and timely records of patient progress and treatment plans
• Participate in quality improvement initiatives to enhance patient outcomes and satisfaction
