Hospice Visit Nurse — 3 days every other weekend 7am‑7pm

Hospice requires compassionate care for patients across 9 counties. The position offers flexible weekend scheduling and competitive benefits.

• Assesses patient needs and evaluates symptoms and progress through the comprehensive assessment of the patient.
• Executes the therapeutic regime of treatments, medications, and plan of care as prescribed.
• Consults and collaborates with the interdisciplinary team and other agencies involved in the patient's care.
• Initiates or facilitates referral of patient and/or family to appropriate agency or community resources.
• Performs admissions as assigned.
• Accurately records and reports pertinent facts and observations regarding the patient's condition and response to treatment; documents on ECF chart when applicable.
• Reports appropriate information to the case manager and the interdisciplinary team, ensuring verbal and written reports are clear, concise and timely.
• Performs other duties that may be assigned.

• Current R.N. nursing licensure in Indiana and CPR certification.
• Prior home health or hospice experience preferred.
