If you became a nurse because you wanted to make a meaningful difference in people’s lives, hospice offers an opportunity to reconnect with that purpose. As a Hospice RN Case Manager, you’ll care for patients where they live, build meaningful relationships with patients and families, and use your clinical judgment to help people live as comfortably and fully as possible. This role offers a different rhythm from traditional hospital or facility nursing.
You’ll work independently in the field, have ownership of your patients and their plan of care, and collaborate with an interdisciplinary team that is there to support you. If you’re a clinically confident, compassionate RN who values autonomy, meaningful relationships and having ownership of your work, hospice nursing may be the change you’re looking for.
- Monday–Friday, day‑shift schedule, with participation in an on‑call rotation
- Greater autonomy and independence in your day‑to‑day work
- Meaningful one‑to‑one time with patients and families
- The opportunity to build relationships and follow patients throughout their hospice journey
- A varied, field‑based environment outside a traditional hospital or facility setting
- Support and collaboration from an interdisciplinary hospice team
- The opportunity to use your assessment, critical‑thinking, communication and education skills every day
- A structured hospice onboarding program, with an experienced hospice nurse mentor to support you as you build confidence in the field
As an RN Case Manager, you’ll be a trusted clinical resource for patients and families while coordinating care with the broader hospice team.
You will:
- Assess each patient’s physical, psychosocial, environmental, safety and spiritual needs
- Develop and manage individualized plans of care based on the patient’s and family’s goals
- Assess and manage pain, symptoms and comfort needs
- Provide skilled, compassionate nursing care in the patient’s residence
- Educate and support patients, families and caregivers as conditions and needs change
- Help patients and families understand medications, hospice services, the plan of care and what to expect throughout the end‑of‑life journey
- Coordinate care and communicate changes in patient needs with physicians and the interdisciplinary team
- Participate in interdisciplinary group meetings to support coordinated, consistent care
- Supervise LPNs and hospice/home health aides in accordance with applicable requirements
- Complete admissions as assigned
- Maintain accurate, timely clinical documentation
- Serve as a resource and preceptor for team members and students, as appropriate
Typical productivity expectation: an average of 20-25 visits per week.
Who Thrives in This Role? We’re looking for more than clinical experience. The nurses who are likely to thrive in hospice are those who enjoy the independence, relationships and personal accountability that come with caring for patients in the community.
You may be a great fit if you:
- Build relationships easily. You take time to understand what’s important to patients and families and earn their trust.
- Are comfortable working independently. You enjoy having ownership of your work and know when to reach out for support.
