Join The Care Team as a Hospice LPN and make a meaningful impact by providing compassionate, hands-on care and support to patients and families during life’s most important moments. We are seeking a dedicated Licensed Practical Nurse to deliver high-quality patient care, support the interdisciplinary team, and help ensure comfort, dignity, and exceptional end-of-life care in home and facility settings.
Schedule, Hours, & Territory- Standard operating hours are Monday through Friday, 8:00 AM to 4:30 PM
- Up to a 1-hour radius of Mishawaka, IN

- Provide nursing care as delegated by the RN
- Monitor patient status and report changes
- Support comfort and symptom management
- Document care provided in the EMR
- Collaborate with the interdisciplinary team
- Active Licensed Practical Nurse (LPN) license
- Hospice or long-term care experience preferred
- Active driver’s license
- Ability to pass a criminal background check
- Ability to pass a Motor Vehicle Record (MVR) check
- Current TB testing in accordance with agency policy
- Current CPR certification as required for the role
