We are currently looking for Registered Nurses, Primary Care to work on a casual (on-call) basis in a number of our Urgent Primary Care Centres at the following locations:

  • Abbotsford - Urgent Primary Care Centre
  • Mission - Urgent Primary Care Centre
  • Chilliwack - Urgent Primary Care Centre

Detailed Overview

In accordance with the British Columbia College of Nurses and Midwives (BCCNM) standards of practice and the Mission and Values of Fraser Health, works collaboratively and as a member of an inter-professional team to provide direct patient care to clients of the Urgent Primary Care Centre (UPCC) or Primary Care Clinic, providing timely and longitudinal primary care to unattached and attached vulnerable clients.

Provides initial client consultation, assessment, coaching and teaching, health promotion and intervention, client care services and follow-up based on the client’s needs and best practices to ensure access to timely, person-centred, continuous and culturally safe quality health services; facilitates and manages client transitions across the healthcare continuum; collaborates with acute, primary and community care providers to facilitate information flow.

Provides client- and family-centered care through a trauma-informed and evidence-informed approach; establishes a dignifying, purposeful and trusting relationship with clients, promoting self-determination and independence; supports clients and families to engage with their community and navigate the health care system.

Responsibilities
  • Provides direct nursing care to clients receiving scheduled or urgent primary care; assesses clients and collects information from a variety of sources; makes a nursing diagnosis and determines appropriate nursing interventions in collaboration with other clinical staff.
  • Processes physician or nurse practitioner orders by initiating standing orders, administering medications, carrying out treatments and monitoring/documenting effects.
  • Establishes a therapeutic relationship with the client to support autonomy in decision-making and care planning, including dignity and privacy.
  • Screens referrals, provides individualized client assessments, care planning and interventions; coordinates with inter-professional team and community resources to optimize function and ensure timely health services.
  • Develops a comprehensive shared client care plan in collaboration with the inter-professional team, client and family; facilitates transitions to primary/community care providers and community agencies.
  • Delivers direct planned and unplanned client care and identifies additional services as required; explains care to client and family.
  • Reviews care plans with the team and initiates next steps to achieve client goals; supports transition across the care continuum.
  • Discusses clinical findings with client/family and develops action plans and goals; plans, prioritizes and responds to changing needs.
