Detailed Overview
In accordance with the British Columbia College of Nurses and Midwives (BCCNM) standards of practice and the Mission and Values of Fraser Health, the Community Health Nurse (CHN) - Registered Nurse works independently in the community setting. Works collaboratively as a member of an interprofessional team, managing an assigned client caseload that includes assessments, coaching, interventions, client care services, and follow‑up to enable clients and their families to live confidently and safely at home and/or in the community.
Emphasizes the promotion, maintenance, and restoration of health, including the treatment of chronic diseases through teaching, counseling, and direct client care. Facilitates and manages client transitions across the healthcare continuum using the provincial Primary & Community Care model, optimizing recovery and minimizing avoidable admissions to residential and/or acute care facilities. Collaborates with acute, primary, and community care providers and family/supports to ensure seamless care planning and transition.
Responsibilities
Establish a therapeutic relationship with the client using interpersonal and interviewing techniques, in person or over the telephone, ensuring the client's choice, autonomy, dignity, and privacy.
Screens referrals, provides individualized assessments, interprofessional care planning, and interventions, including clinical care when appropriate; assists clients to achieve optimal function by facilitating timely and appropriate health services.
Develops a comprehensive shared client care plan in collaboration with the interprofessional team and facilitates its transition to the referring source and community providers.
Provides direct client care, identifies required care services per guidelines, and explains care to the client and family.
Makes decisions on client‑specific direct care tasks as required, delegates tasks to Community Health Workers, provides training, and ensures they operate within defined limits; monitors their performance in partnership with the Community Health Nurse - Licensed Practical Nurse.
Facilitates care conferencing with the interprofessional team to review and adjust care plans, determine referrals, and evaluate effectiveness, developing next steps toward client goals.
Discusses clinical findings with client/family, collaborates on action plans, sets goals, reviews system information, and organizes priorities using resources effectively.
Supports clients and families before, during, and after interprofessional conferences and clinic visits, providing information, advocating for their choices, and delivering direct care during transitions.
Arranges and participates in joint home visits with other professionals, advising on available community resources.
Provides health counseling through education, self‑management, self‑monitoring, and wellness promotion via clinic, telephone, or home visits, collaborating with other health professionals in the primary & community care network.
Maintains clinical records (intake screens, assessments, interventions, care plans, progress notes) and statistical information according to established policies.
Engages in continuous professional practice growth, reviewing literature, attending workshops and in‑services, and networking with health professionals.
