In this role, you'll engage with clients at an Urgent Primary Care Centre, providing assessments, health education, and continuous care planning. Collaborating with inter-professional teams, you will facilitate optimal patient experiences while navigating the healthcare system. Addressing diverse health needs, your nursing interventions will significantly impact clients' health journeys and overall wellness.
Key Responsibilities:
• Provide comprehensive nursing assessments
• Develop and implement client-centered care plans
• Facilitate referrals to community resources
• Support clients in health education and self-management
• Document clinical interventions and outcomes
Requirements:
• Current practising registration as a Nurse
• 1 year of clinical experience in relevant fields
• Valid BC driver’s licence is essential
• Proven ability to communicate effectively
• Skills in prioritizing and managing diverse healthcare issues
