Fraser Health invites you to join an innovative and unique primary health care team in the rapidly growing city of Mission, British Columbia. We are currently recruiting a Nurse Practitioner (NP) to provide comprehensive, integrated, and trauma‑informed care at the Urgent and Primary Care Centre (UPCC) located in Mission, B.C. UPCCs deliver timely, person‑centred, culturally safe, trauma‑informed longitudinal and urgent primary care.

Our model of care offers holistic team‑based care by supporting physicians, nurse practitioners, nurses, social workers, clinical counsellors, allied health professionals and administrative staff. Together, this team extends continuous care that is intentional and coordinated for all community members, including the vulnerable and medically complex.

The role is part of our clinical enhancement program, which includes regular in‑house skills development such as 12‑lead ECG interpretation, point‑of‑care ultrasound (POCUS), suturing, and x‑ray interpretation, alongside a centralized NP orientation. New graduates can join a NP Community of Practice team to support a successful transition into independent practice.

Detailed Overview

The Nurse Practitioner is responsible for comprehensive patient assessment, diagnosis, treatment planning, therapeutic interventions, and medication prescribing in accordance with statutory, regulatory, and organizational standards. The NP provides professional guidance to other health professionals, practices autonomously within an interdisciplinary health care team, and makes specialist referrals when appropriate.

The position collaborates with patients and other health professionals to identify trends and implement evidence‑based strategies to improve community health. The role also participates in peer‑review and self‑review to evaluate service outcomes at patient, community, and population levels.

Responsibilities
  • Diagnose and treat undifferentiated diseases, disorders, and conditions within the NP’s scope of practice; write orders for treatment and medications; provide first‑line care in emergencies.
  • Monitor ongoing care, order appropriate screening and diagnostic investigations, interpret reports, and analyze information to monitor progress and plan treatment.
  • Establish priorities for health management; provide follow‑up treatment; communicate with patients and families about health findings, diagnoses, priorities, outcomes, and prognoses; support and counsel patients in responding to disease.
  • Collaborate and consult with physicians or other health care and social service providers as appropriate; develop and implement treatment plans; may admit and discharge patients according to organizational policies.
  • Prescribe drugs within statutory and regulatory limits for Nurse Practitioners and applicable employer policies.
  • Assign work to nursing and health care personnel; evaluate work and provide education and supervision; hire staff and evaluate performance.
  • Participate in research contributing to improved patient care and advances in nursing, health policy, and population health.
  • Maintain a population health focus by implementing screening and health promotion activities for at‑risk populations.
  • Participate in interdisciplinary staff and nursing education through case presentations, mentoring, role modeling, and knowledge exchange in the classroom, clinical setting, and community; foster health care partnerships.
  • Develop, implement, and evaluate policies and procedures related to nursing, interdisciplinary care, and health system practices.
  • Perform other duties as required.
Qualifications

Education and Experience
  • Current practicing registration as a Nurse Practitioner with the British Columbia College of Nurses and Midwives (BCCNM).
  • Recent relevant clinical nursing experience.
