Holland Christian Homes consists of six independent/Assisted Living apartment towers, as well as Faith Manor and Grace Manor, which are private, non‑profit Christian Long‑Term Care facilities situated in the City of Brampton.

Director of Programs and Services (Faith Manor) & Resident Advocate/Programs Manager (Grace Manor)
Position Summary
Responsible for assisting in the planning, organization and direction of meaningful activities for residents in their prospective Manor.
Major Duties and Responsibilities
Respects and carries out the values associated with a person‑centered approach which include rights, dignity, identity, individuality, respect, privacy, choice and independence.
Provides supportive opportunities for conversation and social engagement to help people live their life and experience well‑being.
Provides sufficient job training, direction and evaluation to placement students completing their practicum hours within the activity department.
Provides direction and guidance to volunteers who are assigned to assist our department with activity programs.
Collaborates and facilitates programs with community stakeholders (including intergenerational activities, musical concerts, animal visits, etc.).
Assists with plant care, gardening and horticultural programs.
Attends monthly department meetings.
Attends and actively participates in interdisciplinary meetings designed to re‑assess the needs of and develop a plan of action for the residents.
Maintains open communication with residents, medical staff, facility staff, families, volunteers and visitors.
Participates in in‑service programs, seminars, conferences and workshops.
Participates in the CQI and Accreditation Program of the facility.
Assesses and develops through planning, organizing, facilitating and evaluating activation programs designed to provide sufficient variety to meet all domains of well‑being.
Uses physical facilities, supplies and equipment effectively and economically.
Is responsible for maintaining all appropriate documentation for i.e.:
Activity Pro, Point Click Care, etc.
Assists with pleasurable dining program (e.g., porter residents to dining room, assist to wash hands and fasten clothing protectors, perform positive attention plan and feed residents as needed).
Participates in daily feeding and care of animals (birds, guinea pigs, rabbits and others), including handling animals, facilitating animal programs and following all IPAC measures.
Prepares and distributes monthly program calendars and assists with planning and facilitating all programs and special events.
Performs other duties and responsibilities as assigned by the direct supervisor.
Participates in Quality Improvement initiatives.
Commits to continuing competence through quality improvement.
Participates in developing and reporting on key performance indicators (KPIs) that support business decision making, and quality improvement plans (QIPs) that are aligned with both legislative and organizational objectives.
Infection Control
Maintains effective infection prevention and control protocols and practices by:
Implementing IPAC practices using best practices that comply with statutory, regulatory and other requirements of the Home.
Engaging in IPAC practices to prevent the spread of infections and protect residents, tenants, staff and others in the facility from infections, resulting in reduced morbidity and…
