Social Worker in Long Term Care Facility

Become a vital part of our Long Term Care facility as a Social Worker, providing essential support and intervention for elderly residents and their families.

Your role will focus on counseling, assessment, and patient advocacy.

You will report to both the Manager of Clinical Services and the Clinical Practice Leader, administering comprehensive social work services. Collaboration with interdisciplinary teams, family support, and crisis management will be central to your duties. A Bachelor’s degree in Social Work and experience in a residential setting with elderly individuals are required.

Key Responsibilities:

• Manage the admissions process and conduct interviews

• Facilitate family support, including financial assistance

• Deliver comprehensive counseling and education to families

• Document as well as maintain records of social work services

• Implement and evaluate quality improvement initiatives

Requirements:

• Baccalaureate degree in Social Work

• Minimum one year experience with the elderly in care facilities

• Current registration with the BC College of Social Workers

• Strong ability to maintain positive working relationships

• Physical capability to perform duties associated with the role
