Position: Social Worker for Community Health Services

Fraser Health is looking for an experienced Social Worker to join its Home Health department. This role involves providing therapeutic and clinical social work services to clients, performing assessments, and collaborating with interdisciplinary teams to deliver quality care. Engaging with clients and their caregivers is crucial for facilitating their access to community resources and services.

Key Responsibilities:

• Deliver therapeutic social work services via assessments and interventions

• Collaborate effectively with interdisciplinary team members

• Offer resource information and referrals to clients and families

• Conduct educational sessions and community outreach for support

• Engage in quality improvement and research initiatives

Requirements:

• Bachelor’s Degree in Social Work and three years’ relevant experience

• Current registration with the BC College of Social Workers

• BC Driver's license and access to your own vehicle

• Strong written and verbal communication skills

• Ability to perform psychosocial assessments and evaluations
