JOB SUMMARYThe Med Care Manager/QMAP must administer, pour, pass, and document all routinely prescribed medications per physician orders under the direction of the Wellness Director and as approved by State Medication Administration Regulations. You will quickly learn how each resident prefers to take their medication and be ready to find the right resident when it’s time for medication.

- Administer and document regularly prescribed medications per the Medication Administration record, observing the five rights for safe medication administration.
- Order medications from the pharmacy 7 days prior to running out and as requested by the Wellness Director.
- Document medications given in the resident’s medical record immediately after administration, including date and initialing the medication pack.
- Observe and report reactions and side effects to the licensed nurse.
- Take and record vital signs before medication administration if required.
- Report any medication error to the Wellness Director immediately.
- Assist with development and review of the resident’s care plan with other disciplines.
- Ensure resident privacy, respect, and dignity.
- Detect, correct, and report unsafe conditions that may harm a resident.
- Assure physical comfort, safety, and mental well‑being of residents.
- Assist in daily tasks such as toileting, grooming, bathing, feeding, dressing, and moving residents.

LICENSE / CERTIFICATIONSuccessful completion of state‑approved Medication Administration training, First Aid certification as required by ALF regulations. QMAP in the state of CO.
EDUCATION / EXPERIENCEMinimum high school diploma or equivalent. Preference for prior healthcare experience, especially in Assisted Living or Memory Care, with a solid work record and medication administration experience. Ability to make resident‑centered decisions and support company policies.
CONTINUING EDUCATIONAttend monthly in‑service education and training within the community as required by law.
OTHERMust read and write English, interpret safety rules and operating instructions, communicate effectively, and provide resident‑care services within delegated tasks. Exhibit good judgment in recognizing changes in resident condition. Work with licensed nurses and respect limitations of role.
JOB KNOWLEDGEPerform medication administration functions, understand required procedures, and apply universal precautions to prevent infection.
INDUSTRY EXPERIENCEMinimum three years’ experience in senior living preferred, especially in an Assisted Living community.
Team members scheduled to work 24 or more hours per week are eligible for benefits; families of such team members are also eligible.
