Horizon Health in Mission, SD is seeking a Full Time Community Health Worker to educate and connect patients with identified social determinates of health to available resources and assist with navigating barriers. We are looking for candidates who are passionate about delivering high-quality, patient-centered care. Horizon Health offers a collaborative, team-oriented work environment focused on meeting the needs of individual patients while supporting and contributing to the health and wellness of our communities.
This is a fully onsite in clinic position.
Key Responsibilities:- Health system navigation and resource coordination.
- Health promotion and coaching providing information or education to patients that makes positive contributions to their health status.
- Health education to teach or promote methods and measures that have been proven effective in avoiding illness and/or lessening its effects.
- Provide culturally appropriate health information and education to patients.
