JOB SUMMARY
The Prevention Specialist provides one-on-one preventive care support to individuals and families, with a focus on chronic disease prevention, health education, and navigation of health and social services. This role works collaboratively with participants and partners to co-develop personalized Prevention Plans, address social determinants of health, and support progress toward improved health outcomes.
DUTIES AND RESPONSIBILITIES
Provide preventive care support to individuals and families, including education on chronic disease risk factors and preventionConduct holistic needs assessments and co-develop personalized Prevention Plans with participantsFacilitate goal setting and action planning using motivational interviewing and behavior change approachesConnect participants to relevant community resources, programs, and preventive services, and support service navigationConduct follow-up sessions to monitor progress, adjust Prevention Plans, and support sustained behaviour changeCollaborate with internal teams, healthcare providers, and community partners to ensure coordinated and culturally responsive careMaintain accurate documentation, referrals, and progress notes to support reporting and program evaluationQUALIFICATIONS
University degree in a health-related field (e.g., health promotion, public health) or equivalent experienceMinimum two years of experience in community health, preventive care, or a related fieldKnowledge of chronic disease prevention and health behavior change approachesStrong interpersonal and communication skillsAbility to manage multiple priorities, work independently, and collaborate within a teamExperience working with diverse communities and understanding of social determinants of healthHOW TO APPLY
