Key Responsibilities- Collaborate on creating personalized care strategies
- Provide emotional and symptom management support
- Educate families on palliative care principles
- Attend clinical meetings and discussions for care coordination
- Travel within your designated area to meet patients
- Registered and in positive standing with the College of Nurses of Ontario
- At least two years of nursing experience required
- Palliative care experience preferred
- Additional certifications in palliative care advantageous
- Current CPR certification and drivers license required
