Parkdale Nurse Practitioner - Outreach Focus

Key Responsibilities:

• Facilitate substance use treatment and care plans

• Conduct health assessments and diagnostic tests

Requirements:

• Registration as an RN (Extended Class) with Ontario's nursing board

• 3 to 5 years of relevant nursing experience

• Strong knowledge in harm reduction and mental health care

• Effective communication and problem-solving abilities

• Ability to work in diverse, multi-disciplinary environments
