Supervisor, Integrated Care Services in Palliative

Spectrum Health Care is searching for a transformative Registered Nurse (RN) or Registered Practical Nurse (RPN) to guide an integrated team in providing high-quality palliative care. Key Responsibilities:

• Supervise day-to-day operations of the palliative care team

• Collaborate on developing and reviewing care plans

• Monitor and support healthcare professionals’ coordination

• Manage quality and safety issues within service delivery

• Build trust with patients and their families for effective care

Requirements:

• Valid RN/RPN registration with the College of Nurses of Ontario

• At least 5 years of community nursing experience

• Experience in palliative care and program implementation

• Relevant palliative care certification or courses

• Valid G driver's license and be available for on-call
