Partners Community Health (PCH) is a new not‑for‑profit organization, focused on bringing healthcare services together around the needs of people living in Mississauga and West Toronto. PCH currently operates two new state‑of‑the‑art LTC homes and a community hub in West Mississauga. The new homes have a combined 632 beds and are part of PCH’s larger strategy to introduce innovative and inclusive programs and services and new models of care delivery.

As a Registered Practical Nurse (RPN) you will support PCH residents by assuming the following responsibilities:
Community Driven
Establishes and maintains good interpersonal relations with colleagues, residents, families, volunteers, and the community
Caring Service
Collaborates with other members of the care team
Assumes responsibility for medication administration, narcotic control, removal of discontinued, unused, or expired medication, application of treatments and dressings, transcription of Physician orders, reordering of medications, preparation and other skills
Promotes the PCH's philosophy of care
Promotes and supports the Resident’s Bill of Rights
Completes other duties as assigned
Professionalism
Is accountable to the Nursing leadership team for providing nursing care and clinical intervention for the care and services delivered to the Residents on the assigned RHA
Functions in accordance with Guidelines for Ethical Behaviour in Nursing
Participates in the Home Area programs (e.g. Occupational Health and Safety Program, Quality Monitoring and Analysis Program, and Fire Safety).
Promotes a co‑operative working environment
Demonstrates a commitment to the Mission, Vision, and Values of PCH
Provides input to the development and revision of Personal Care policies and procedures and communicates changes to staff
Facilitates team building and promotes change and innovative ideas
Uses scientific knowledge in a systematic approach to identifying problems and in problem solving
Seeks out opportunities to maintain competency and to continuously learn, taking an active role in continuous quality improvement
Qualifications
Post‑graduate studies in Geriatrics and Nursing Administration is an asset
Current certification of competence of the College of Nurses of Ontario
Current Medication Certificate which meets the College of Nurses Standards of Practice
One year experience in a long‑term care facility with knowledge and interest in gerontology
Members R.P.N.A.O./R.N.A.O./Gerontological Nursing Association preferred
Applicants must successfully pass the prescribed Vulnerable Sector Check
Job Requirements
Member of CNO with Active RPN License
Valid First Aid & CPR Certification
Must present a clear Vulnerable Sector Check (completed in the last 6 months)
Completed 2-Step TB Test
Must be available to work weekends and statutory holidays
