Program Manager for Integrated Care

Lead integrated care delivery as a Program Manager with Spectrum Health Care in Mississauga. This 6-month contract allows you to manage multidisciplinary teams while driving operational and clinical excellence.

Spectrum Health Care is seeking a Registered Nurse as a Program Manager, Integrated Care. In this role, you will lead a team of Nurses and Support Workers, fostering innovation and collaboration. Your focus will be on optimizing service delivery through data insights and coordinating with health system stakeholders to enhance care transitions.

Key Responsibilities:

• Lead program design and clinical workflow development

• Recruit, onboard, and mentor team members effectively

• Monitor quality, safety, and risk issues proactively

• Contribute to client outcomes with evidence-based practices

• Collaborate with stakeholders for innovative program implementation

Requirements:

• Current CNO certificate as Registered Nurse

• At least 5 years in Nursing Manager roles

• Experience in managing union environments preferred

• Strong understanding of leading multidisciplinary teams

• Valid G driver's license and access to a vehicle
