Key Responsibilities:

• Create personalized care plans with the interdisciplinary team

• Educate and support patients and their families

• Conduct clinical huddles and case conferences

• Adhere to CNO standards and Spectrum policies

• Travel as needed to various patient locations

Requirements:

• Registered nurse in good standing with CNO

• At least two years of nursing experience

• Palliative care experience preferred

• Valid driver's license and willingness to travel

• Current CPR and First Aid Certification
