Geriatric Nurse for Outreach Services

We are seeking a Registered Nurse to collaborate with the Geriatric Psychiatry Outreach services. This position requires two years of geriatric mental health nursing experience, offering specialized interventions and education to staff in Long-Term Care homes. You will act as a consultant, develop care strategies, and facilitate knowledge transfer to improve the quality of life for older adults with complex needs.

Key Responsibilities:

• Establish relationships with Long-Term Care staff

• Provide tailored psychiatric nursing interventions

• Assist in developing and monitoring behavioral care plans

• Facilitate staff education on responsive behaviors

• Support coordination between healthcare teams

Requirements:

• Registered Nursing Diploma;
Baccalaureate preferred

• 2+ years in geriatric mental health nursing

• Current registration with the College of Nurses of Ontario

• CNA certification in Psychiatry preferred

• Excellent critical thinking and communication skills
