As a key member of Ontario’s largest home health care team, you will provide critical nursing support to patients of all ages. Ideal candidates will be Registered Nurses (RNs) or Registered Practical Nurses (RPNs) who thrive in collaborative settings. This position involves developing care plans and delivering personalized nursing services across various environments.

Key Responsibilities:

• Develop Care Plans respecting community health needs

• Deliver exemplary nursing care in-person and virtually

• Work in homes, clinics, and schools

• Communicate effectively with patients and care teams

• Perform clinical tasks as necessary

Requirements:

• RN or RPN certification

• Current registration with CNO

• Strong patient care commitment

• Excellent verbal and written communication skills

• A valid driver's license for travel
