Registered Practical Nurse in Palliative Care

Key Responsibilities:

• Work under Integrated Care Program Supervisor

• Create personalized care plans with team input

• Participate in clinical huddles and case discussions

• Ensure compliance with health care policies

• Travel to various patient residences and facilities

Requirements:

• Licensed and in good standing with CNO

• Minimum two years nursing experience required

• Experience with palliative patients preferred

• Valid Ontario driver's license and vehicle

• Current certifications in CPR and First Aid
