Nurse Practitioner

Position: Nurse Practitioner at Partners Community Health
Provide high-quality care as a Nurse Practitioner at Partners Community Health in Mississauga. This role emphasizes direct patient care for residents with complex health needs within an interdisciplinary team environment.

At Partners Community Health (PCH), the Nurse Practitioner serves in long-term care facilities, focusing on patient assessments, diagnosis, and management. You will use your advanced clinical skills to collaborate with physicians, nurses, and other specialists, ensuring comprehensive care for residents. The position is rooted in quality improvement practices and patient-centered care principles.

Key Responsibilities:

• Formulate differential diagnoses through resident assessments

• Perform comprehensive physical examinations as needed

• Order and interpret diagnostic investigations

• Prescribe appropriate pharmacotherapy for residents

• Communicate and educate residents and families on health outcomes

Requirements:

• Bachelor’s Nursing degree;
Master's preferred

• Current Ontario Nurse Practitioner registration

• 5 years of gerontological nursing experience preferred

• Knowledge of Ontario healthcare standards

• Proven leadership, communication, and interpersonal skills
