This role focuses on facilitating exceptional patient support services and improving pharmaceutical commercialization. As a Nurse Case Manager, you will provide top-tier assistance, manage case protocols, and ensure effective communication among patients, families, and healthcare teams. Your contributions will directly impact healthcare outcomes and build relationships across Cencora’s services.
Key Responsibilities:
• Support patients with medication access and inquiries
• Document patient interactions within the electronic health record
• Collaborate with healthcare providers for therapy logistics
• Train new Nurse Case Managers on processes
• Attend relevant meetings and conferences for knowledge sharing
Requirements:
• Licensed Nurse (RN or RPN/LPN) with active registration
• 2 to 5 years in healthcare or pharmaceutical settings
• French and/or English fluency as required
• Experience in case management and patient support
• Strong analytical and problem-solving skills
