Lead integrated care operations as a Registered Nurse in Mississauga, using clinical leadership and data-driven insights to improve community services.
Spectrum Health Care seeks a Program Manager, Integrated Care to lead Nurses, Personal Support Workers, and Allied Health professionals. This senior nursing leadership role manages program development, clinical workflows, service quality, and stakeholder collaboration. You will support innovative care models and seamless transitions across the health system.
Key Responsibilities:- Lead program development and design clinical workflows
- Recruit, onboard, and develop multidisciplinary team members
- Mentor teams toward consistently high care standards
- Monitor performance indicators and initiate process improvements
- Address quality, safety, and risk issues
- Current CNO Registered Nurse certificate; degree preferred
- At least five years of recent progressive nursing management experience
- Community nursing management experience preferred
- Program development, implementation, and evaluation experience
- Valid G driver's license and reliable vehicle
