Position: Nurse Practitioner – WellFort Health Program
Join Well Fort's initiative as a Nurse Practitioner, part-time, in Mississauga, ON. Address the healthcare needs of those facing homelessness with vital support and services.

In this crucial role, you will provide advanced primary care, ensuring equitable access to health services for the structurally vulnerable populations in our community. Collaborate with an interprofessional team to deliver comprehensive medical care including mental health support, chronic disease management, and health education tailored for newcomers and their families.

Key Responsibilities:

• Conduct patient assessments and formulate diagnostic plans

• Prescribe medication and monitor treatment adherence

• Develop and implement culturally safe health strategies

• Advocate for clients to mitigate barriers to service

• Maintain accurate EMR documentation and participate in audits

Requirements:

• Registered Nurse with a recognized NP certification

• 3-5 years of relevant nursing experience, especially in community settings

• Knowledge of social determinants affecting health

• Proficient in EMRs and Microsoft Office

• Valid driver's license and reliable vehicle for outreach
