Position: Outreach Support Worker for KDFN
Join Kwanlin Dün First Nation as an Auxiliary On-Call Outreach Support Worker in the Health and Wellness Department. Provide compassionate support to clients and foster community wellness.

KDFN is seeking dedicated Outreach Support Workers who excel in culturally safe approaches. Ideal candidates will leverage their knowledge of socio-economic factors and residential school impacts while connecting community members with essential resources. This role requires a commitment to bridging gaps through education and referrals while supporting individuals and families.

Key Responsibilities:

• Educate clients about available healthcare services

• Build trusting relationships within the community

• Offer appropriate referrals to necessary resources

• Document outreach activities and interactions

• Collaborate with multi-disciplinary care teams

Requirements:

• Basic education in social sciences or addiction field

• Two years experience in outreach services

• Knowledge of Yukon First Nations socio-economic history

• Ability to maintain strict confidentiality

• Valid Class 5 Drivers License with a clean record
