** - Embrace the chance to make a real impact on patients’ lives in their own homes.
- Evaluate and establish tailored care plans to meet the unique needs of each patient.
- Collaborate with a multidisciplinary team to coordinate care and enhance patient outcomes.
- Utilize evidence-based practice guidelines and Medicare regulations to deliver high-quality therapy.
- Master the art of OASIS scoring and contribute to quality improvement initiatives.
- Maintain a basic understanding of insurance requirements in Home Health.
- Engage in stimulating multidisciplinary conferences to create relevant, patient-centered care plans.
- Document all skilled services, adverse events, and communications with precision during point of care.
- Manage productivity standards and ensure effective use of your time and resources.
- Communicate daily with the scheduler to maximize continuity of care and efficiency.
- Supervise Occupational Therapy Assistants and assist in providing top-notch therapy services.
- Plan admissions, OASIS time points, and discharges effectively throughout each episode of care.
- Uphold the highest ethical standards as defined by the licensing board and agency policies.
- Participate in engaging educational opportunities provided by the agency.

* Minimum Qualifications:
** - Bachelor's degree or higher from an accredited Occupational Therapy program.
- Current license to practice in the State of Montana.
- Valid driver’s license and appropriate auto insurance.
- Two years of previous OT experience preferred.
