Dover Health, a regional healthcare provider and true friend of adults facing serious illnesses, seeks a service-minded, career-oriented individual to join our team as a Home Health / Hospice Community Liaison at our St. Louis location.

As a Home Health / Hospice Community Liaison, you will impact the lives of our patients by –
- Analyzing the potential of the company’s service area to determine target markets.
- Visiting doctor offices, hospitals, Assisted Living facilities, Skilled Nursing Facilities, and other sources of referrals to present agency credentials and obtain patient referrals.
- Analyzing the company’s organization to determine its strengths and weaknesses.
- Analyzing past and current marketing data.
- Completing an analysis of the company’s service line.
- Analyzing patient/company relationships.
- Developing a marketing plan, identifying priorities, and setting a reasonable timeline.
- Implementing the marketing plan within the established timeline.
- Reviewing and evaluating the analysis and plan regularly.
- Coordinating referrals through all Dover Health services.
- Attending IDG meetings as appropriate.
- Serving as the first contact from clinical referral sources.
- Providing initial informational meetings with patients and/or families.
- Assisting office staff with obtaining signatures and updates as needed.
- Providing community outreach and education.
- Assessing patients’ and families’ ability to cope with the patient’s dying.
- Functioning as a consultant to the health team, helping them understand social, emotional, and environmental factors related to the patient’s health problems.
- Helping patients utilize available resources for their families and community.
To become a Dover Health Home Health / Hospice Community Liaison, you will need –
- At least 1 year experience in home health or hospice sales/marketing.
- Clinical license and experience preferred.
- Must be a licensed driver with an insured automobile in good condition, a clear driving record, and current liability auto insurance.
- Willingness to travel locally and occasionally overnight.
- Excellent verbal and written communication skills.
- Ability to meet all physical demands of the job, including seeing, speaking, touching, walking, standing, bending, reaching overhead, crouching/kneeling, crawling, and lifting/moving up to 50 lbs independently and up to 200 lbs with assistance.
- Ability to work well with others, take direction, and show initiative to ensure patient needs are met.
- Self-motivated with a strong sense of accountability for results.
