IHART is a mobile community resource team comprised of non-police personnel that will provide timely response – with a targeted response time of 10-12 minutes in the city of Vallejo. IHART will provide welfare checks, screening, and assessments to link residents to resources based on their specific and urgent mental and behavioral health needs. IHART mobile community resource unit will provide trauma-informed responses and linkages for mental and behavioral health and social services related calls, thus reducing the role of police and improving outcomes for individuals experiencing mental health- and social services-related crises.
IHART will have partnerships with community system partners, to include, but not limited to, hospital emergency departments, Fire/EMS departments, SCBDHS’ mobile crisis team to provide 12-hour alternative response services to low-level 911 calls from Vallejo Residents. IHART response will focus on de-escalation, mitigation, and prevention of repeated emergency situations. Implementation of the IHART Program will build a collaborative network of service providers to allow for a safer response to behavioral health, quality of life and low threshold medical calls.
Data collection activities from the IHART Program will support opportunities for evaluation and inform strategies for improvement.
Under the direct supervision of the IHART Program Manager, the Mobile Community Crisis Intervention Worker, MCT will respond to a broad range of non-life-threatening medical calls and situations without police or EMS personnel 12 hours per day, 7 days per week citywide. Mobile Community Crisis Intervention Worker will provide on-site crisis assessment to provide aid to community members in crisis. MCT will support linkages that can address resident’s goals, including engaging in mental health, substance use treatment and when needed utilize the services of the IHART Behavioral Health personnel, Resource Team member or another provider.
MCT will act as a liaison with community partners and EMS agencies in addressing resident’s needs while also conducting outreach and may provide non-emergency transport to necessary service locations so that engaged Vallejo residents can begin a relationship with a provider immediately. The position requires strong interpersonal skills to make connections and build relationships with community residents in need.
- Conduct Care Coordination and Engagement using established protocols and procedures to ensure clients served are connected to Mental Health Services, e.g., impatient and outpatient scheduling.
- Provide low-threshold case management services for residents with substance use, mental health, and/or other social service needs and other salient challenges.
- Ability to work a flexible full-time schedule to accommodate program/client needs. Both evening and weekend hours are required to accommodate a 12hr/7 day a week schedule. This is a community based, highly mobile position.
- Maintain documentation of programmatic activities, response calls and assists in tracking data related to program activities.
- Maintain inventory of health education/outreach materials necessary to conduct outreach efforts and to supply internal providers and other staff as needed.
- Provide hands-on support for clients in reaching and accessing services according to their intervention plan (medical and social service appointments, assist in some activities of daily living, such as grocery shopping) and/or assist clients in connecting with and accessing available transportation resources.
- Complete assessments, service plan development, and service linkage (housing navigation, treatment and sobering centers, medical, and other basic needs and support)
- Assist residents in implementing their wellness and recovery plans and goals to establish continuity of care via Short-Term, Standard and Extended Follow Ups.
