Job Overview
Social Service Practitioner II and III – Hospital Social Service Practitioners provide assessment, intervention, and case management support to individuals, groups, children, and families with complex health, psychosocial, or economic problems. They investigate and support cases involving neglect, abuse, emotional or behavioral problems, physical/mental disabilities, or other health conditions, and often assist clients with emergency situations and risk assessment.
Minimum Requirements- Social Service Practitioner II

Education:
Master’s degree in Social Work (MSW) from a Council of Social Work Education‑accredited school.

Experience:
None required. - Social Service Practitioner III
•
Education:
Master’s degree in Social Work (MSW) from a Council of Social Work Education‑accredited school.
•
Experience:
Three (3) years as a Social Service Practitioner in San Bernardino County or equivalent experience.
• Required documentation:
Copy of MSW degree or official transcript.
