Social Worker (MSW/LCSW) – Full Time – Marina del Rey Hospital

Responsibilities
  • Perform social work assessments and interventions for inpatient and Emergency Department patients.
  • Provide comprehensive assessments and develop plans integrating medical and social resources.
  • Conduct crisis intervention, patient/family intervention, high‑risk screening, brief counseling and refer to resources.
  • Establish and maintain professional relationships with physicians, case managers, nursing staff, county agencies, community resources, patients and patient representatives.
  • Assure integration and coordination of social service/discharge planning activities within the hospital system.
  • Develop specific, measurable, realistic plans and objectives to enhance social service delivery.
  • Perform psychosocial assessment and reassessment on all patients including pediatric, adolescent, geriatric and general populations.
  • Document patient care in accordance with current standards and policies.
  • Interact professionally with patients/families, involving them in the plan of care.
  • Communicate clearly with physicians and ancillary services; consult other departments to collaborate in patient care.
  • Coordinate and direct social services to meet patient needs and comply with hospital policy.
  • Provide resources and support to staff.
  • Execute consultative activities and recommend service delivery changes; act as liaison for Advance Care Planning, complex discharge planning, and biopsychosocial assessments.
  • Assess behavioral health patients and arrange evaluations, 5150 holds, psych transfers, and follow‑up.
  • Attend team rounds and participate in interdisciplinary collaboration; assist with recommendations from psychiatry and neuropsychology.
  • Use clinical judgment to address psychosocial barriers, coordinate discharge for complex cases and under‑resourced patients.
  • Other duties as assigned.
Qualifications Education
  • Master's Degree in Social Work (minimum).
Work Experience
  • At least 2 years of clinical social work experience (preferred).
  • Preferred 2 years in a hospital‑based medical setting, palliative or hospice care.
  • One year of MSW internship may be considered; discharge planning experience preferred.
