General Pay Scale: $22.00 - $30.24 (actual rate dependent on experience, education, responsibilities, location, performance)
OverviewThe Patient Navigation Coordinator provides individualized assistance to patients, families, and caregivers to overcome healthcare system barriers and access resources throughout the cancer care continuum—from prevention to end-of-life care. Under the guidance of Social Workers, this role supports patients by providing resources, education materials, form completion, and health-related screenings while ensuring compliance with all standards and regulations.
Responsibilities- Connect patients with resources and services for practical needs.
- Communicate with physicians, advanced practice providers, nurses, social workers, and other members of the care team.
- Transfer/escalate situations when appropriate with accurate information utilizing resources to resolve inquiries within the scope of the department.
- Travel intermittently to Northern Clinics in Albany and Corvallis for in-person patient support.
- Document patient interactions and services in the EMR system.
- Act as a liaison and advocate for patients and caregivers.
- Participate in process improvement initiatives to enhance patient outcomes and satisfaction.
- Maintain strict patient confidentiality in compliance with HIPAA and organizational policies.
- Perform other duties as assigned.
- High school diploma or GED; advanced education preferred.
- Minimum of four (4) years of experience in a clinical health care environment required.
- Medical Assistant (certified) experience preferred.
- Health care-related education, planning, management, or similar experience preferred.

Requires full range of body motion including handling and lifting patients, manual and finger dexterity, and eye‑hand coordination. It requires standing and walking for extensive periods of time. Occasionally lifts and carries items weighing up to 40 lbs. Requires corrected vision and hearing to normal range.
The work environment may include exposure to communicable diseases, toxic substances, ionizing radiation, medical preparations and other conditions common to an oncology/hematology clinic environment. Work will involve in‑person interaction with co‑workers and management and/or clients. Work may require minimal travel by automobile to office sites.
