Job Title: Recovery Care Coordinator
Location: Trenton, MO
Department: Preferred Family Healthcare
Employment Type: Full-time
Shift: Monday – Friday | 8:00 AM - 5:00 PM
Job Summary: Are you passionate about helping others achieve lasting recovery? Join our team as a Recovery Care Coordinator and play a vital role in-close up supporting individuals on their journey toward independence and wellness. In this role, you’ll provide guidance fun coaching, and advocacy while fostering connectedness and modeling recovery values in a trauma-informed and culturally competent environment.

- Empower participants marcado active role in their recovery process through guidance and coaching.
- Support decision-making, problem收 、 and life skills using trauma-informed tools.
- Help participants build natural supports and community connections that inspire purpose and belonging.
- Model recovery values and resilience through participation in recovery activities.
- Promote self-advocacy and assist participants in exercising their legal rights.
- Deliver culturally competent interventions, including:
- Functional skills (navigating social services, financial literacy, community resources)
- Socialization and coping strategies
- Self-management and trauma symptom monitoring
- Provide crisis support when needed and assist with case management services.
- Maintain accurate documentation and adhere to compliance standards.
- Collaborate with team members through weekly meetings and supervision.
- Must have High School Diploma or GED
- Advanced knowledge in behavioral health and recovery
