Job Details:
- Evaluate assigned patients and develop, implement, and modify treatment plans in accordance with patient needs and physician direction
- Delegate and direct all discipline-specific therapy services for assigned patients
- Design functional treatment programs to support effective return to community activities
- Serve as an educator for patients, families, students, and coworkers
- Complete comprehensive evaluations and utilize clinical reasoning to develop appropriate treatment plans and goals focused on progressing patients toward discharge
- Collaborate with interdisciplinary team to address all barriers to discharge, including equipment trialing and ordering, patient and family training, and planning for continuing services
- Manage individual, concurrent, and group treatment sessions based on patient care needs
- Complete medical record documentation in a clear, concise, accurate, and timely manner including daily documentation, goal writing, functional quality indicator assessments, letters of medical necessity, discharge documentation, and charge entry
- Ensure care is provided under signed, valid physician orders and within insurance approval
- Participate in student program as assigned, providing a positive learning environment with appropriate supervision
- Adhere to discipline-specific State Practice Act and other regulatory agency requirements
- Maintain professional licensure in good standing and complete all state-mandated continuing education requirements
- Minimum of 2 years of experience required
- BLS Certification required prior to first shift
- Must hold a valid, current state license
