Position Summary:
The Document Control Analyst serves as a subject-matter expert responsible for maintaining the facility’s medical records and is responsible for the accuracy, safety, and confidentiality of the records. The Document Control Analyst is responsible for a variety of tasks, including but not limited to, document imaging operations and software, collecting patient information, issuing medical files, filing medical records, and processing resident admissions and discharge papers.
This position requires a basic knowledge of medical terminology, effective communication, customer service, and problem-solving skills. The Document Control Analyst reports directly to the Health Information Management Manager and works closely with the hospital department and medical staff to foster a Culture of One.
Essential Duties and Responsibilities:
- Regular and reliable attendance is a fundamental qualification and essential function of this position without compromising operational standards and team collaboration.
- Responsible for the collection, processing, filing, maintenance, storage, retrieval, and distribution of medical records according to facility policies and procedures.
- Scans and organizes medical record information for processing, including but not limited to, incorporating documentation into the electronic medical record, sorting and entering mail, and chart tracking.
- Handles incoming telephone calls and voicemails from patients, health care professionals, and payers, ensuring they are answered with appropriate follow-through in a timely manner.
- Handles Release of Information for continuing care following departmental policies and procedures.
- Performs data entry and answers emails.
- Achieves consistent quality performance and meets the quality standard as defined by the department.
- Achieves consistent productivity performance and meets production standards for the specific job role.
- Must be able to perform the professional, clinical and or technical competencies of the assigned unit or department.
- Effective written and oral communication with internal and external customers while adhering to the HIPAA standards of practice. This includes but not limited to answering phone calls, responding to portal inquiries, and departmental communications.
- Utilize critical-thinking skills to resolve inquiries of a non-clinical nature accurately and with the proper LRHS resources.
- Performs departmental duties and/or projects as assigned that are within the scope of the department’s functions.
Required
Education/Certification:
- A High School diploma or equivalent required.
Required
Knowledge, Skills and Abilities:
(Experience)
- 2+ Years of HIM healthcare experience.
- Prior experience with an electronic medical record (EMR), Meditech EMR desirable.
- Strong computer, data entry and office management skills
- Scheduling experience within a fast-paced environment is strongly preferred.
- Strong customer service skills to interact positively with internal and external customers
- Exceptional communication skills with both patients and clinical staff to ensure information is transferred accurately and timely.
- Ability to juggle and prioritize multiple responsibilities and handle interruptions
- Strong organizational skills to keep patient information confidential and organized
- Problem-solving skills for scheduling conflicts, missing documentation or issues as they arise
- Attention to detail to ensure all patient information is accurate and available
- Compassion to help patients and caregivers in difficult situations
- Understanding of the importance of confidentiality
