• Base Pay Range: $22.00 - $34.74 per hour, based on experience

• Identify and maintain an up-to‑date directory of resources within patients’ local communities to address individual social determinants of health. Connect patients with the appropriate services to support health and wellness.
• Identify transportation resources and assist with coordination for patients to optimize appointment attendance.
• Assist patients with determining eligibility for Medicaid coverage and submission of required documentation.
• Identify and problem‑solve patient obstacles for compliance with an identified treatment plan or patient‑directed care goals.
• Engage regularly with care teams to better understand patient needs and how to best assist with their non‑clinical needs.
• May complete unit/department‑specific duties as outlined in department documents.

• High school diploma or an equivalent combination of education and experience from which comparable knowledge, skills, and abilities can be acquired.
• Community Health Worker (CHW) certification by the Missouri Credentialing Board (MCB) within one (1) year as a condition of continued employment in this job classification.
• Two (2) years of experience in a healthcare setting.
