Licensed Practical Nurse – Home Health

As a full‑time Home Health LPN in Osage Beach, you will:

• Provide professional nursing services under the direction of an RN in compliance with state regulations and Center Well Home Health policies.
• Provide skilled nursing interventions for patients’ illnesses, rehabilitative needs and preventative care, using a holistic approach.
• Apply clinical knowledge to facilitate problem resolution and achieve individualized patient goals.
• Consult with the RN supervisor regarding needed changes in the plan of treatment, accept verbal orders from physicians as permitted by law, and communicate these orders to the RN supervisor.
• Use a systematic, goal/outcome‑driven approach in implementing the nursing plan of care.
• Maintain timely communication with patients, physicians, clinical managers and other healthcare team members regarding patient/family needs and status changes; participate in care coordination activities and discharge planning.
• Maintain professional conduct with confidential information, sharing information only when permission is established and in the best interest of the patient.
• Ensure the availability of supplies, materials and equipment needed to safely and effectively implement the plan of care.
• Prepare, submit and maintain documentation as required by the Company; document daily shift notes for services rendered.

• Graduate of an accredited Licensed Practical Nursing Program or accredited School of Vocational Nursing.
• Current nursing license in the practicing state.
• Valid driver’s license, auto insurance and reliable transportation.
• Current CPR certification.
• Minimum one year experience as an LPN/LVN in a clinical setting, preferably home health or hospice.

Pay Range:

• $28.00 – $40.00 per visit/unit
• $44,600 – $61,400 per year base pay

Scheduled weekly hours: 40 hrs.
