Location: California
Licensed Vocational Nurse (LVN), Skilled Nursing
- 1 year experience monitoring and caring for clients/residents preferred
Certifications:
- Must possess a current unencumbered State of California license
- American Heart Association BLS
Required:
- Ability to communicate clearly in both oral and written forms using the English language.
- Ability to respond to common inquiries or complaints from residents, patients, physicians, peers, regulatory agencies, or members of the business community.
- Neat/professional appearance.
- Demonstrate competence in taking vital signs, body systems assessments, giving enemas, measuring I & O’s accurately, and recording results accurately and legibly.
- Respond to patient needs and requests promptly.
- Must demonstrate proficiency in administering medications using standard methods.
- Extensive knowledge of the use of equipment and other medical supplies.
- Evaluate equipment and apply the latest techniques.
- Maintain a positive and caring relationship with patients, residents, families, and co-workers.
- Care for the geriatric patient demonstrating clinical skills and knowledge of the aging process.
- Comply with established procedures.
- Support and promote safety standards, identify risk situations, and intervene appropriately.
- Ensuring that utility rooms are maintained in a neat and orderly fashion.
- Adhere to infection control policies and protocols, medication and storage procedures, and controlled substance regulations.
- Maintain current knowledge of universal precautions in dealing with potential exposure to blood-borne pathogens.
- Initiate and maintain confidentiality, privacy, and safety for all patients.
- Monitor CNAs’ to ensure compliance with Hospital and HIPPA policies and procedures as they apply to privacy.
- Provide timely and accurate information to staff for the next shift.
- Monitor staff performance to ensure compliance with all policies and procedures regarding patient care.
- Conduct in-service training.
- May assist in administering the annual competency testing program.
- Coordinate MDS activities and reviews.
- Facilitate interdisciplinary rounds/meetings to identify and resolve patient/family-related issues that may affect the discharge outcome.
- Maintain the Compliance Matrix (DHS Survey Preparation) ensuring that all information is available.
- Schedule Care Plan conferences.
- Monitor all admissions to ensure compliance with established procedures and protocols and conduct follow-up reviews for QA purposes.
- Identify patient/family discharge needs and provide support services and/or referrals to support services and agencies as required.
- Order equipment for home use and initiate follow-up calls to monitor patient condition.
- Perform other job-related duties as assigned.
