The Nurse Care Manager utilizes best practice guidelines in compliance with the mission/philosophy, standards, goals and core values of Brown and Toland Physician’s; to impact utilization including hospital admits, reducing readmissions, and increasing preventive care compliance with the of goal improving patient outcomes. Addresses care coordination, resource management, and may provide authorization review and coordination
** This position is a hybrid opportunity in the Bay Area/East Bay, and you must be able to travel up to 50% of the time within the region.
**** Your focus will be on:
*** Utilize clinical knowledge, problem solving and decision-making skills to evaluate requests for authorization, verifying eligibility, interpretation of benefits and medical necessity
* Creates positive partnerships with primary care providers, specialists, hospital staff, community agencies, and other key partners
** The Skills, Experience & Education You Bring
*** Current California unrestricted RN license
* Associate Degree in Nursing from accredited program
* Minimum 2 years acute clinical experience in hospital setting
* Minimum 2 years care management experience (hospital or community based) providing direct patient care
* Minimum 5 years professional experience
* Managed Care experienced
** Base Salary**$55.00 - $63.00/hr
