Hospice Nurse Practitioner - PRN

LHC Group is hiring a Nurse Practitioner in hospice care at Community Loving Care Hospice in Wentzville, MO.

We are seeking a dedicated Nurse Practitioner to provide palliative, symptom management, and supportive care to hospice patients as requested by the referring physician.

• Makes face‑to‑face visits within prescribed time limits to qualifying hospice patients and reports clinical findings to the Hospice Medical Director.
• Consults with the Hospice Medical Director as needed and informs primary physicians of recommended services.
• Initiates and coordinates the plan of care, documents problems, goals, interventions, and patient/family response to hospice care.
• Instructs and supervises patients/families in self‑care techniques when appropriate.
• Maintains accurate clinical notes and reports patient condition to Administrator and Hospice Medical Director.

• Minimum of five years nursing experience; clinical nursing care in oncology, hospice, geriatrics, intensive care preferred.
• Current certification as a clinical Nurse Specialist or Nurse Practitioner in the state of practice.
• Current certification by a national body such as ANCC, NBCHPN, or AANP.
• Current driver’s license and reliable vehicle or public transportation.
• Current CPR certification required.

• Palliative Care ARNP preferred.
• Knowledgeable regarding interdisciplinary team concepts.
