Advanced Primary Care RN II (Non-Exempt)
The RN II Advanced Primary Care role provides virtual care to patients that benefit from ongoing chronic disease management. In partnership with a multidisciplinary team, including physicians and advance practitioners, the RN is responsible for creating patient relationships, monitoring vital sign trends, conducting patient visits in between regular office visits, assisting in panel management of patients and providing individualized care to improve patient outcomes.
Reports to the respective Advanced Primary Care Lead APP.

Is personally responsible for obtaining, and maintaining, a current RN license within the hiring state and/or compact licensure in which nursing duties are performed and must meet all state board of nursing requirements. Must be eligible for licensure in other states.
Education and ExperienceGraduation from an accredited registered nursing program. Bachelor’s degree in nursing (BSN) or equivalent work experience. Minimum 5 years of healthcare experience required. Previous telemedicine, internal/family medicine, or care management experience preferred.
