PositionSummary
Tru Health seeks an advanced practice provider—either a nurse practitioner (NP) or physician assistant (PA) with experience in primary care or internal medicine, preferably with a geriatrics focus—to improve outcomes for long‑term care residents. The role collaborates with providers and case managers, works primarily within nursing homes or assisted‑living communities, and is responsible for personalized, coordinated health care that improves quality of life and prevents unnecessary hospital visits.
The provider exercises independent judgment to treat patients with acute and chronic conditions, consulting with a supervising physician as appropriate, and implements our national model of care in compliance with all federal and state regulations.
- Assess patients’ medical and mental health needs and provide a proactive plan of care that addresses barriers to care
- Ensure compliance with local, state, and federal agencies related to clinical services
- Prescribe medications and order lab work, diagnostic procedures, and consultations
- Monitor patients’ compliance and response to treatment and modify plans as needed
- Work with RN case managers as part of an integrated care team
- FNP, AGNP, AHACNP or PA license required
- Degree from an accredited APRN or PA program
- Three years of clinical nursing or rehab experience in geriatric populations
- Experience with Electronic Health Records
- Proficiency in Microsoft Word, Outlook, and Excel
- May be required to provide training and advice to facility staff
