About the Role

The RN Care Ambassador is the clinical lead and a primary point of contact for clients and families. You will complete comprehensive assessments, build and manage personalized care plans, coordinate across providers and the in‑home team, and ensure that every interaction reflects the Legacy standard: precise, calm, and deeply human.

Responsibilities
  • Assessment and Planning
  • Complete comprehensive in‑person (or virtual) intake assessments, including medical history, current concerns, psychosocial dynamics, and client preferences.
  • Answer client and family questions with clarity and empathy; set expectations and next steps.
  • Complete clinical documentation and an intake summary report within established time frames.
  • Monitoring and Evaluation
  • Communicate effectively with clinical and non‑clinical staff, clients, families, and clinical personnel in a courteous, compassionate, and efficient manner.
  • Conduct weekly client visits (or as clinically indicated) to reassess care needs and provide guidance to the in‑home team.
  • Apply strong problem‑solving and decision‑making skills: identify root causes, implement corrective action plans, and evaluate effectiveness.
  • Coordination of Care
  • Create and maintain individualized care plans in collaboration with the client and/or family.
  • Coordinate across the in‑home team and external providers to support continuity of care and a seamless client experience.
  • Participate in after‑hours and weekend on‑call coverage on a rotating basis to support client and family needs.
  • Client Advocacy and Communication
  • Provide ongoing phone and email communication with families, primary care physicians, power of attorneys, and the care team.
  • Serve as a liaison between the client, family system, and surrounding social systems, collaborating with professionals and non‑professionals involved in care.
  • Support transitions of care (hospital‑to‑home, post‑op recovery, new diagnoses) with clear planning and follow‑through.
  • Quality and Compliance
  • Ensure compliance with all applicable rules, regulations, standards, and internal policies.
  • Complete clinical quality tracker audits and support continuous improvement initiatives.
Qualifications
  • Active California Registered Nurse (RN) license.
  • 5+ years of nursing experience; 3+ years of clinical/bedside nursing experience.
  • Prior hospital, home health or case management experience required; experience with geriatric populations required.
  • Strong assessment skills and clinical judgment; confident communicator in high‑stress situations.
  • Excellent verbal and written communication; high‑quality documentation habits.
  • Familiarity with dementia and chronic diseases common to the aging population.
  • Process‑driven and organized; able to reset priorities and thrive in a fast‑paced environment.
  • Proficient with Microsoft Office and Google Workspace tools.
  • Willing to be field‑based and travel throughout Los Angeles County; reliable transportation required.
  • BSN required; MSN preferred.
