LPN / RN MDS Nurse Coordinator
Key Responsibilities:
Conduct and coordinate Resident Assessment Instruments (RAIs) per federal, state, and local regulations.
Collaborate with the Interdisciplinary Care Plan Team to develop resident-specific care plans.
Evaluate resident conditions and ensure all special assessments are completed as required.
Provide data for the Facility's Quality Assurance Program.
LPN / RN MDS Nurse Coordinator
Qualifications:
Active and current LPN or RN license in the state of Missouri
Previous experience as an MDS Coordinator
Strong understanding of Medicare/Medicaid regulations and benefit guidelines
