Licensed Practical Nurse (LPN)

Job Summary: The Licensed Practical Nurse (LPN) provides compassionate, patient-centered nursing care under the direction of a Registered Nurse or physician. The LPN administers medications and treatments, monitors patient conditions, performs nursing procedures, documents care, and collaborates with the interdisciplinary healthcare team to promote safe, high-quality patient outcomes.

• Graduate of an accredited Practical Nursing program.
• Current Licensed Practical Nurse (LPN) license in the state of practice.
• Current BLS certification preferred or required per facility policy.
• Recent experience in long-term care (LTC), skilled nursing (SNF), rehabilitation, clinic, or acute care preferred.

• Provide direct nursing care in accordance with individualized care plans.
• Administer medications and treatments as prescribed.
• Monitor, assess, and report changes in patient condition to the Registered Nurse or provider.
• Assist with admissions, transfers, discharges, and ongoing patient care.
• Maintain accurate and timely clinical documentation.
• Collaborate with physicians, nurses, and the interdisciplinary healthcare team.
• Educate patients and families within the scope of practice.
• Follow infection prevention, patient safety, HIPAA, and regulatory standards.
• Perform other duties as assigned.
