AboutPATH

About the JobPATH’s CalAIM program provides services to participants receiving services through managed care plans. Through these partnerships, staff support Medi‑Cal beneficiaries experiencing homelessness, including chronic homelessness, who have at least one complex physical, behavioral or developmental health need. The Case Manager’s main function is to assist clients in breaking the cycle of homelessness using a “whatever it takes” approach, supporting them through all phases of homelessness: on the street, interim housing, transitional housing, and permanent housing.
